Call or visit website to locate a provider. Bring photo ID, AHCCCS card, Social Security card, list of medications, and name of family doctor to first appointment.

Description:

Provides an AHCCCS Health Plan for central and southern Arizona. Partners with a system of providers that deliver behavioral health and medical care to people who are signed up for AHCCCS/Medicaid. Offers non-emergency medical transportation to and from covered medical and behavioral health appointments. This service is available for members who are unable to provide or pay for their own transportation. Website offers a database of providers searchable by location and service. Accepts and attempts to resolve complaints.

Is UnitedHealthcare community plan AHCCCS?

UnitedHealthcare Community Plan is an Arizona Medicaid health plan serving AHCCCS Complete Care, KidsCare, and Developmental Disabilities (DD) members. We have nearly 40 years of experience serving families throughout Arizona.

How do I check my AHCCCS status?

Phone Verifications: Maricopa County: 602-417-7200. Outside of Maricopa County, within Arizona: 1-800-331-5090.

Does AHCCCS cover gym membership?

AHCCCS CARE Account fund purchases are limited to non-covered services. At this time, approved services include: dental, vision care, nutritional counseling, recognized weight loss programs, chiropractic care, gym membership and sunscreen.

Is AHCCCS Medicare or Medicaid?

AHCCCS is Arizona's State Medicaid Program. AHCCCS Members who also have Medicare are called Dual Eligible Members. Being enrolled in the same health plan for Medicare and Medicaid is called “alignment.”
