The Center for Vital Records (birth, death, marriage, and adoption certificates) is now open by appointment only at it's new location in Simpson Hall on the Pastore Campus (6 Harrington Rd., Cranston).

Apply for certified copies of birth certificate, death certificate, marriage certificate, or divorce certificates. Keep in mind that not all offices offer every service, and some offices have a limited date range available. Every Health Department office has different fees and processing times. You may order birth and death certificates by phone with a credit card by calling VitalChek at 1-877-297-9125. All major credit cards are accepted, including American Express®, Discover®, MasterCard® or Visa®. The VitalChek fax number is 1-888-985-2842.

Can you get a birth certificate same day in Florida?

First photocopy certificate (same day walk-in service requires $10 rush fee, when available). Lobby hours: M-F 8:00 a.m. – 4:30 p.m. Walk-in payments may be made in the form of cash, check, money order, Visa, and Mastercard.

Where can I get a Florida birth certificate?

The Florida Department of Health keeps Florida birth and death certificates. You may obtain certificates either by contacting your local Department of Health Office or the Florida Department of Health's Bureau of Vital Statistics located in Jacksonville, FL.

Are birth certificates public record in Florida?

Birth certificates are not public record: Access is limited to individuals 18 years of age or older who would like a copy of their own birth certificate and to parents or guardians (who present court documents verifying legal custody) who would like copies of their children's birth certificates. Cost is $16 per copy.
