Caregiver Support ProgramThe Caregiver Support Program enables caregivers to maintain their quality of life and culture while caring for their loved one at home. The program provides a spectrum of services including care consultation, support groups, respite services, telephone support, and access to services Dayhaven Social Adult Day ProgramDayhaven is a social model adult day program for individuals with short and long-term physical and/or cognitive impairments. The program provides an interactive, safe, and secure place for individuals to make new friends, share a meal, exercise their brain, learn new activities, and connect with others. Dayhaven allows the caregiver peace of mind while they are at work or doing daily activities and know their loved one is being taken care of in a safe, secure environment. Health Insurance Information Counseling & Assistance ProgramThe Health Insurance Information Counseling & Assistance Program (HIICAP) provides Medicare beneficiaries with information, education, and counseling about Medicare and other health insurances. HIICAP staff and volunteers serve seniors in Schenectady County with non-biased, confidential, and free counseling. Long Term Care Ombudsman ProgramThe Long Term Care Ombudsman Program (LTCOP) provides opportunities for residents of long term care facilities and their families to voice problems and concerns that impact their quality of life and care. The role of the LTCOP is important in helping ensure the safety and welfare of thousands of New York State long term care residents. A Long Term Care Certified Ombudsman is a dedicated, trained staff or volunteer committed to improving the quality of care of residents in New York State long term care facilities. Home Delivered MealsThe Home Delivered Meals Program, aka Meals on Wheels, serves homebound individuals and those who are not able to get their nutritional needs met on their own. At the core of the Meals on Wheels Program is a nutritious meal providing one-third of the recommended dietary allowances delivered to homes by staff or volunteers who keep a watchful eye on the well-being of our seniors. Nutrition Outreach and Education ProgramThe Nutrition Outreach and Education Program's (NOEP) nationally recognized model is one of the largest Supplemental Nutrition Assistance Program (SNAP) outreach programs in the nation. SNAP is a federally-funded program that stretches the monthly food budgets of all who meet program guidelines. NOEP offers free, confidential, one-on-one services that connect Schenectady County residents with nutrition assistance programs. Senior Dining Centers - Friendship CafeFriendship Cafes' are gathering places for seniors where a nutritious mid-day meal is served with others. Seniors enjoy meeting with friends and participating in diverse activities that offer seniors the opportunity to remain active and connected to the community while meeting their nutritional needs. Transportation ProgramThe Transportation Program is dedicated to providing user-friendly services to seniors age 60 and older. Services are rendered by compassionate staff utilizing a small fleet of buses and mini vans with wheelchair accessibility. Weekday services are available to medical appointments, senior dining centers, and Dayhaven Social Adult Day Program.
